Summary

The facility failed to ensure safe smoking practices were followed for supervised residents who smoke. During multiple observed smoking times, surveyors saw residents using one cigarette to light another, handing lit cigarettes to other residents, flicking ash onto the patio or ground, and extinguishing cigarettes on wheelchairs. Surveyors also observed activity staff members smoking or vaping while they were supposed to be supervising the smoking area, and at one point the designated smoking area was left unattended and unsupervised for approximately four minutes while residents remained there with access to a lighter. The facility’s own staff acknowledged that supervised smokers were not supposed to be left unattended, that staff were responsible for lighting residents’ cigarettes, and that residents were not supposed to use their own cigarettes or other residents’ cigarettes to light additional cigarettes. The Administrator and DON also stated that cigarette ashes and butts had to be disposed of in an ashtray and that staff should not be smoking while supervising residents. Resident interviews confirmed that residents were aware of the smoking rules but continued to use their cigarettes to light others, and that staff did not consistently intervene when this occurred. Resident #58 was admitted with diagnoses including executive function deficit following a stroke and nicotine dependence, and was cognitively intact on the most recent MDS. The resident’s smoking assessment and care plan identified that he/she dropped ashes on self and required a smoking apron on the lap while smoking. During observation, Resident #58 was seen smoking in the designated area with two staff members present but without the required smoking apron. Staff later stated that the apron was required every time the resident smoked and that it had been forgotten during the observed smoking period.
