Failure to Document Insulin Administration and Implement Physician Orders

Summary

The facility failed to ensure care was provided in accordance with professional standards of practice for two residents. For one resident with diagnoses including hyperglycemia and diabetes, the record showed an order for Humalog sliding scale insulin before meals and at 8 P.M., but the documentation in the progress notes and MARs did not consistently include the 8 P.M. blood sugar levels, the number of Humalog units administered when insulin was required, or the site of administration. The resident’s records also reflected ongoing diabetes management concerns, including an elevated A1C, an increase in Glargine, continued sliding scale insulin, and later addition of Tradjenta after blood sugars remained in the 300s. For that same resident, staff interviews confirmed the missing documentation. A nurse stated the electronic record should populate fields for blood sugar, insulin dose, and site of administration for sliding scale insulin, but those entries were not documented. The Unit Manager and DON also reviewed the order and confirmed that the 8 P.M. blood sugar levels were not documented on the MAR and that the order appeared to have been entered incorrectly. For another resident with dementia with behavioral disturbance and depressive episodes, the consultant pharmacist recommended medication changes, including reducing Citalopram and discontinuing or placing a stop date on PRN Trazodone. Although the physician signed the recommendation forms, the active physician orders in the record still showed Citalopram 10 mg daily and PRN Trazodone without a stop date, and the MARs reflected those unchanged orders. In addition, the resident had a prosthetic left eye, and an interim/telephone order for weekly prosthetic eye care was written in the paper chart but was not dated or signed by a receiving nurse, did not appear on the active physician orders, and was not documented as administered on the MARs.
