Summary

The facility failed to ensure medical records were complete and accurate by not keeping evidence of completed pharmacy recommendations in the resident record for one resident out of a sample of 26. The report states that the consultant pharmacist reviewed the resident’s medication regimen monthly and repeatedly documented the same recommendation to review the risk versus benefit of Amitriptyline, but the physician and NP notes from August 2025 through April 2026 did not document that risk versus benefit discussion. The electronic and paper medical record also did not show that the pharmacist’s recommendation had been addressed by the physician. The resident was admitted in December 2023 with diagnoses including adjustment disorder with mixed anxiety and depressed mood, type 2 diabetes mellitus with polyneuropathy, and chronic kidney disease stage 4. Current physician orders showed Amitriptyline HCl 10 mg at bedtime for anxiety. Monthly pharmacy reviews from August 2025 through April 2026 repeatedly listed the same MD recommendation regarding Amitriptyline, yet the record did not contain documentation showing completion of that recommendation. During interviews, unit management stated the pharmacy recommendations were emailed to the DON, distributed to unit managers for completion, and then returned to the DON office, where they were kept rather than placed in the medical record. The covering DON acknowledged the recommendations were not part of the medical record and were unavailable for the consultant pharmacist to review each month. The lead pharmacy consultant stated the repeated recommendation suggested it had not been addressed or was not part of the medical record, and the Administrator stated the completed recommendations should have been in the record and available for pharmacy review.
