Improper Sling Size Used During Mechanical Lift Transfer

Summary

The facility failed to ensure that a resident who required transfer with a mechanical lift and two staff members was provided the correct size sling pad during a bed-to-wheelchair transfer. Resident #1 had diagnoses including dementia, low back pain, lumbar spondylosis with radiculopathy, atrial fibrillation, hypothyroidism, hypertension, and insomnia. The quarterly MDS indicated severe cognitive impairment with a BIMS score of 06 out of 15, and the care plan identified dependence on transfers with a Hoyer lift. On the morning of the incident, two CNAs used a Hoyer lift to transfer the resident from bed to wheelchair. The CNAs reported using an orange sling pad, and both later stated that the resident had usually been transferred with a blue mesh sling pad, which they described as the size normally used for the resident. During the transfer, the lift base struck the wheelchair as the lift was turned, the resident swung back and forth, and then slid out through the bottom opening of the sling pad onto the floor, landing on the right side. The resident complained of generalized pain, and the pain level was documented as 10/10. Nursing assessment found no visible injury, but the resident was sent to the hospital emergency department for evaluation after the provider was notified. Interviews with the nurse, CNAs, and DON confirmed that the orange sling pad was larger than the blue mesh sling pad typically used for the resident, and the DON stated that the orange sling pad was too big for the resident. The report also noted that the nurse did not know whether the lower straps had been crisscrossed and did not know the size of the orange sling pad used during the transfer.
