Summary

Facility staff failed to ensure Minimum Data Set (MDS) assessments were accurately coded for 3 residents reviewed during a complaint survey. The report states that the MDS is part of the Resident Assessment Instrument and is used to identify each resident’s individual needs and guide care planning. During review of facility reported incident #3013380 and medical records, surveyors found that Resident #4 had a fall on 5/12/26 while being assisted in the bathroom and later had nose swelling, a bruise-like area on the nose, and bruising around both eyes, yet the Discharge Return Anticipated MDS with ARD 5/13/26 coded Section J1900B as 1 fall with no injury. The MDS Coordinator stated it was not a major injury and later confirmed the error. Surveyors also found that Resident #17 had a fall from a wheelchair with forehead swelling, a mild headache, and an order to send the resident to the ER for a CT scan of the head, but the quarterly MDS with ARD 5/29/26 coded no falls since the prior assessment. The MDS status was export ready, indicating it had been completed and locked. In addition, Resident #18 had an unwitnessed fall, was found on the floor beside the wheelchair, reported dizziness before falling, and stated being in and out of consciousness with fluctuating blood pressures, but the 5/25/26 Discharge Return Anticipated MDS coded Section J1800 as no falls. The MDS Coordinator confirmed both MDS errors.
