Failure to Follow IV Hydration and Wound Care Orders

Summary

The facility failed to ensure care was provided in accordance with physician orders for two residents. One resident, admitted with diagnoses including atherosclerotic heart disease, hypertension, chronic kidney disease, and chronic systolic heart failure, had a care plan for dehydration/fluid maintenance and was noted to be hypotensive and in acute distress. A nurse practitioner gave verbal orders for STAT labs and IV hydration, and the physician order later documented 1 liter of normal saline IV at 75 cc/hour and insertion of a midline for IV hydration. The resident’s record showed the IV fluids were not administered until the following day, after the order had been initiated, and the NP stated the IV was intended to start on the day the order was given. During the same review, the surveyor observed the crash cart on the unit and reconciled the log with the contents. Although the log indicated all required items were present, the cart was missing three IV starter kits, one primary IV tubing set, and three IV catheters in varying sizes. These items were marked as available on the checklist but were not actually present. Staff interviews indicated the assigned nurse and unit manager could not locate an IV starter kit when the resident needed IV fluids, and the ADON stated a third-party company ultimately started the IV because facility nursing staff were unable to do so. A second resident with dementia, hypertension, peripheral artery disease, and chronic kidney disease had a venous/arterial ulcer and a care plan directing staff to follow the wound provider’s orders. The physician order required the right lower medial leg wound to be cleaned with normal saline, patted dry, treated with calcium alginate, covered with border gauze, and wrapped with Kerlix every evening shift. During observation, the ADON performed wound care but covered the wound with an abdominal pad instead of the ordered border gauze. The ADON stated the order was not followed because another nurse had read it and believed the treatment matched the written order, and the DON stated the ADON should have checked the wound order personally.
