Summary

The facility failed to revise care plans after changes in resident condition and failed to conduct care plan meetings after completion of comprehensive and quarterly assessments. For Resident #5, the record showed a broken tooth was identified by the dentist on 12/9/25, with a progress note on 12/11/25 stating the resident was referred for extraction and that the MD was aware. When the resident’s care plan was reviewed on 1/15/26, it did not include a problem related to the broken tooth. The MDS nurse confirmed the care plan should have contained that problem, and the DON also confirmed the care plan lacked any entry for the broken tooth. For Resident #8, the resident was observed without natural teeth and stated they were waiting for dentures that had already been made, while cutting food into tiny bits to eat. The dental record showed the resident’s remaining teeth were extracted on 5/23/25, followed by dental follow-up visits and impressions for complete upper and lower dentures, with wax impressions fitted and adjusted on 9/30/25. Review of the active care plan on 1/20/26 did not reveal a problem for edentulous status or any indication the resident was in the process of obtaining dentures. The MDS nurse confirmed there was no care plan entry for the resident’s lack of teeth, and the DON acknowledged the issue. For Resident #62, the record showed an admission assessment and quarterly assessments with ARDs of 5/8/25, 8/8/25, 9/17/25, and 12/18/25, but the last documented care plan meetings were held on 5/16/25, 8/5/25, and 12/16/25. The resident stated they did not know whether care plan meetings were held regularly or when care changed. Interviews with Social Services, the resident assessment coordinator, and facility leadership showed conflicting understanding of how care plan meetings were scheduled, and the Social Services Director stated she did not schedule a care plan meeting after the 9/17/25 quarterly assessment unless specifically told to do so. The NHA, Regional DON, and DON reviewed the findings and acknowledged the concern.
