Inaccurate Controlled Medication Documentation and Improper Narcotic Destruction
Summary
The facility failed to maintain accurate documentation for controlled medications administered to residents by keeping the Medication Administration Record (MAR) consistent with the controlled substance record (narcotic book). For one resident, the narcotic book showed Morphine Sulfate oral solution was administered on 6/01/2026 at 3:00 AM, but the MAR did not document that dose. For another resident, the narcotic book showed Oxycodone HCl 10 mg was administered on 6/05/2026 at 4:30 PM, but the MAR lacked documentation of that administration. An RN stated that controlled medications were expected to be documented in both the narcotic book and the MAR immediately after administration. The facility also failed to follow proper pharmaceutical procedures for destruction of discontinued controlled substances. During review of a narcotic count sheet for a resident whose oxycodone order had been discontinued, surveyors found the sheet marked through with a diagonal line but with no licensed nurse signatures or witness verification showing the medication had been destroyed. The narcotic sheet reflected 26 tablets received, several doses administered, and 22 tablets remaining after the order was discontinued. The resident’s MAR showed the medication had been administered as ordered during the period reviewed, leaving the remaining tablets to be destroyed. Staff interviews and record review showed the facility could not explain or verify the destruction of the remaining oxycodone tablets. The Interim DON stated that medications are wasted or destroyed by two licensed nurses who document their signatures or initials on the narcotic count sheet and another sheet sent to unit managers. A unit manager stated that discontinued narcotics are to be documented on a destruction sheet and forwarded to the controlled substances office, but could not explain the absence of documentation for this resident’s discontinued medication. The investigation also noted that an audit of narcotic count sheets and shift count sheets was to be conducted for 12 weeks and reported to QAPI, but the last audit identified was in April 2026 and there was no indication the results were reported to QAPI.
