Summary

Safe, appropriate pain management was not provided for a resident who required pain services. Resident #139 reported pain and was treated with oxycodone, first as a PRN order for moderate to severe pain and then as routine dosing for persistent pain. The resident’s pain medication was later decreased from 4 times per day to 2 times per day because of drowsiness. A pain assessment documented that the resident had reported inconsistent administration of oxycodone and that a routine oxycodone order was placed for follow-up. The record also showed that pain monitoring was completed at least once per shift, but the resident was unable to communicate pain level most of the time. Staff stated that non-verbal pain assessment included signs such as grimacing during care and moaning, and that the computer would indicate whether the resident was communicative or non-communicative. Progress notes documented that the resident was drowsy and opened eyes when called by name, and a change of condition assessment was completed for altered level of consciousness. Although the resident had an order for non-pharmacological interventions before PRN pain medication, there was no documentation that these interventions were implemented before narcotics were administered.
