Summary

The facility failed to make a reasonable effort to substitute an alternative meal when a resident declined the lunch tray. During observation on 04/20/2026 at 1:25 PM, Resident #193 told GNA #15 that the food was too hard and that they did not eat fish, and requested an egg salad sandwich instead. GNA #15 did not contact the kitchen for an alternative meal at that time. At 1:31 PM, the surveyor informed Unit Manager #7, who went to the room and heard the resident repeat the request for an egg salad sandwich; the Unit Manager noted that the GNA had omitted the request even though the resident was hungry. Record review showed Resident #193 was admitted in 2022 with a history of stroke, unspecified dementia without behavioral disturbance, and anxiety disorder. The resident’s prior quarterly BIMS score was 12, indicating moderately impaired cognition. Dietary records showed a diet order dated 11/12/2025 with a daily 2 PM snack and nutrition notes indicating weight monitoring. The kitchen lunch meal ticket for 04/20/2026 listed an alternative for egg salad but did not specify the no-fish restriction. During interview, the resident stated they were unaware that alternative trays could be requested, and the Unit Manager stated the resident had lived in the facility for over four years but remained unaware of available alternative trays and staff had not made any effort to offer them even when prompted.
