Untrained caregiver fed resident while bed was nearly flat

Hebrew Home Of Greater WashingtonRockville, Maryland Survey Completed on 04-23-2026

Summary

The facility failed to ensure that residents received care from competent, properly trained nursing staff in accordance with professional standards of practice. This deficient practice was identified for one resident who was being fed while lying in bed at approximately a 15-degree elevation by a care provider seated beside the bed. During the observation, the resident was heard yelling, "my stomach hurts." When asked whether the resident's head should be elevated during feeding, the care provider stated, "no." The resident identified the individual as a caregiver who regularly feeds and provides care, and the caregiver confirmed that she was not a GNA or CNA. She stated that she provided care for the resident until 12:00 PM, when another agency staff member took over. GNA #10 confirmed that the resident had a private duty sitter scheduled until 12:00 PM. The DON stated that residents should only be assisted with eating by facility-trained staff.

F0726 F726: Ensure that nurses and nurse aides have the appropriate competencies to care for every resident in a way that maximizes each resident's well being.
Short Summary

Licensed nursing staff did not have competencies completed on hire, and the facility did not ensure staff had the skills needed to carry out resident care and monitor changes in condition. One resident’s positive urine culture was not reported to the NP for days, and another resident did not receive ordered BID BG checks; abnormal lab results and a recommendation for IV fluids were documented, but no follow-up was found in the record. The facility assessment listed competency areas such as change in condition and BG testing, and the Regional RN confirmed new-hire competencies were not being completed.

F0726 F726: Ensure that nurses and nurse aides have the appropriate competencies to care for every resident in a way that maximizes each resident's well being.
Short Summary

An LVN failed to follow a resident's oxycodone order by administering the medication when documented pain scores were below the ordered parameter. The LVN could not explain medication parameters or what to do when a resident was outside the parameter, and the DON stated that medication parameters are used for resident safety and that not following them can cause complications, medication toxicity, and ineffective treatment.
