Incomplete Clinical Records and Missing MAR/TAR Documentation
Summary
The facility failed to ensure that clinical records were complete and accurate for one resident reviewed for falls and for three residents reviewed for MAR/TAR documentation. For the resident with a fall history, the record showed an unwitnessed fall and a telehealth provider instruction to monitor per facility protocol with neurological checks, but the Neurological Evaluation Flow Sheet was missing four neurological evaluations. The facility policy required neurological evaluations after an unwitnessed fall, including checks every 15 minutes for two hours, then every 30 minutes for two hours, then every 60 minutes for four hours, and then every 8 hours until at least 72 hours had elapsed. For one resident, the MAR/TAR lacked documentation for multiple ordered interventions and treatments, including vital signs, wound monitoring, wound treatment for coccyx MASD, Tubi grip application, swallowing monitoring during med pass, non-pharmacological pain interventions, and wound treatment with Aquacel Ag and Coflex zinc lite. The record also lacked evidence that several other ordered tasks were completed, including monitoring of pain, air mattress checks, head-of-bed assessment, fluid restriction documentation, application of petroleum and Micatin cream, monitoring for antidepressant side effects, monitoring of cellulitis and socially withdrawn behaviors, CHF education, weights, wound monitoring, wound treatments, and wrapping of the lower left leg. For the other resident, the MAR/TAR also lacked documentation for multiple ordered care tasks, including petroleum application to the feet and toes, pain assessment, air mattress checks, head-of-bed assessment for shortness of breath, fluid restriction documentation, acknowledgment of a topical pain relief order, Micatin cream application, monitoring for antidepressant side effects, wound monitoring and wound treatment for cellulitis, groin, perineal area, and other wounds, monitoring of socially withdrawn behaviors, non-pharmacological pain interventions, CHF education, vital signs, weights, and wrapping of the lower left leg. During interviews on 6/2/26, the Market Clinical Advisor confirmed the missing neurological evaluations and the MAR omissions.
