Infection Control Program and Hand Hygiene Failures

Summary

The facility failed to maintain an Infection Control Program designed to provide a sanitary environment and help prevent the development and transmission of disease and infection during a dining service observation. During lunch service in the dining room, a CNA observed a resident with clear nasal drainage. The CNA put on gloves, used a tissue to wipe the resident’s nose, discarded the tissue, removed and discarded the gloves, and then, without sanitizing her hands, put on a clean pair of gloves and began to feed the resident by lifting the cover on the resident’s meal plate. When the surveyor intervened, the CNA stated that she sometimes forgets to wash her hands and that she would do it then. The facility also failed to conduct an annual review of its Infection Prevention and Control Program (IPCP). Review of facility policies showed multiple infection control-related policies, including Infection Prevention and Control, Vaccination of Residents, Influenza Vaccine, Pneumococcal Vaccine, and isolation policies, with review dates in 3/2025. The Infection Prevention and Control policy stated that policies and procedures are reviewed and revised as necessary and at least annually. During interview, the DON confirmed that these policies were the most current and stated she was unaware that they needed annual review.
