The facility failed to meet residents’ nutritional needs in accordance with established national guidelines by not following the posted menu and by not providing an ordered nutritional supplement. On 06/01/2026 and 06/02/2026, the posted lunch menus near the dining area listed specific meal items, including dessert and cornbread on one menu and dessert on the other. Resident #1 had diagnoses including Alzheimer’s disease, dysphagia, anxiety, pain, and dementia, had a BIMS of 00, severely impaired cognition, and required substantial/maximal assistance with eating. His physician orders included a regular diet with pureed texture and thin liquids. During observation on 06/02/2026, Resident #1’s lunch tray contained pureed chili and pureed vegetables, but no pureed dessert or pureed cornbread as listed on the menu. The private sitter stated this was how the kitchen sent the tray, and an LPN confirmed the resident was not served all meal items even though he should have been. Resident #2 had diagnoses including Alzheimer’s disease, dementia, and dysphagia, had severely impaired cognition, and required partial/moderate assistance with eating. Physician orders included a frozen nutrition supplement every 24 hours for weight loss with lunch, along with a regular diet with pureed texture and thin liquids. On both 06/01/2026 and 06/02/2026, Resident #2 was observed in the dining room with a private sitter during lunch, and the resident was not served the ordered frozen nutrition supplement. The private sitter stated she had never seen the resident receive the supplement with lunch, and the Dietary Cook confirmed after reviewing the dietary card that Resident #2 did not receive the frozen nutrition supplement. The DON confirmed that Resident #1 should have received all menu items and Resident #2 should have received the ordered supplement, but had not.
