The March 2026 MAR documented that the resident received Lorazepam 0.5 mg, two tablets by mouth on 03/05/2026. On 03/25/2026, the DON confirmed the PRN psychotropic medication had been ordered beyond 14 days with no stop date.

The DON reviewed the MAR and stated it was not accurate because staff had different ideas of what restlessness was, and stated the resident should have the correct manifestation of anxiety reflected in the physician order and MAR. The facility policy stated psychotropic medications are to be used only when a practitioner determines the medication is appropriate to treat the resident’s specific, diagnosed, and documented condition.
