Summary

Provide appropriate pressure ulcer care and prevent new ulcers from developing was not ensured for a resident admitted without pressure ulcers. The resident had diagnoses including Alzheimer's disease, dementia, moderate-protein calorie malnutrition, and anorexia. The admission MDS dated 02/04/2026 showed the resident was admitted without any pressure ulcers and required moderate assistance to roll left and right and substantial to maximal assistance for chair/bed-to-chair transfers. A Braden Risk assessment identified the resident as at moderate risk for pressure ulcer development. The resident's skin evaluation note dated 03/10/2026, completed by the DON, documented no skin issues with the resident's heels. The next day's skin evaluation note documented a deep tissue injury to the right heel, described as 3.5 cm x 3.0 cm and 100% dark, observed during am care. During interview, the DON stated she did not assess the resident's heels during the skin evaluation on 03/10/2026. The DON also stated she could not provide documentation that the resident's heels were being floated and that heel boots were being used before the deep tissue injury developed, and confirmed the order for floating the heels was written after the injury was identified.
