Hand Hygiene Not Performed During Incontinence Care

St Joseph Of HarahanHarahan, Louisiana Survey Completed on 06-17-2026

Summary

The facility failed to ensure staff completed hand hygiene during incontinence care for 2 residents observed by surveyors. The facility’s Hand Hygiene policy stated hand hygiene shall be performed when staff hands move from a contaminated body site to a clean body site during patient care, and CDC guidance similarly stated staff should decontaminate their hands when moving from a contaminated body site to a clean body site during patient care. For one resident, a CNA removed a soiled diaper, wiped feces from the resident’s buttocks and perineal area, and then continued care without changing gloves or performing hand hygiene before applying powder, placing a clean diaper, rolling the resident, and covering the resident with clean linen. For another resident, two CNAs removed a soiled diaper, wiped feces from the resident’s buttocks and perineal area, handed the soiled diaper to the other CNA to discard, and then continued with clean diapering, repositioning, and covering the resident without changing gloves or performing hand hygiene. One CNA also touched the resident’s television remote control, bedside table, drink container, and newspaper with contaminated gloved hands. The CNAs stated they did not change gloves or perform hand hygiene after handling the soiled brief and before handling clean items, and the CNA supervisor, DON, and Administrator acknowledged the findings.
