Summary

Provide safe and appropriate respiratory care for a resident when needed was not ensured for 2 residents who required respiratory support, including tracheostomy care and tracheal suctioning. Facility policy for oxygen administration stated that oxygen tubing and masks/cannulas should be changed weekly and as needed if soiled or contaminated, delivery devices should be kept covered in a plastic bag when not in use, and equipment cleaning and care should follow facility policy. Resident #8 had diagnoses including cerebral infarction, non-traumatic intracerebral hemorrhage, tracheostomy status, and epileptic seizures related to external causes, and had an order for suctioning as needed at bedside and every shift for secretions. On 06/15/2026 and again on 06/16/2026, Resident #8's suction tool was observed on the bedside table unbagged, and an LPN confirmed it should have been bagged. Resident #15 had diagnoses including COPD with exacerbation, obstructive sleep apnea, pneumonia due to klebsiella pneumonia, and diastolic CHF. Orders included changing tubing, mask and/or nasal cannula weekly, cleaning the mask daily, cleaning tubing weekly, using BIPAP every night shift, and emptying and air drying the water chamber daily. On 06/15/2026 and 06/16/2026, Resident #15 was observed wearing oxygen via nasal cannula, with the BIPAP mask lying on the bedside table unbagged, the nasal cannula dated 06/07/2026, the humidifier dated 06/06/2026, and the oxygen concentrator filter showing a white powdery substance. The WCN acknowledged the BIPAP mask should be cleaned daily and placed in a bag, the nasal cannula and humidifier should have been changed weekly, and the filter should be taken out and cleaned weekly. The ADON confirmed there was no documentation that the BIPAP equipment was cleaned as ordered.
