Failed Pest Control Program With Ongoing Fly Activity
Summary
The facility failed to maintain an effective pest control program. The facility’s Pest and Insect Control Policy & Procedure stated that an effective, regular basic pest control program was to be maintained, garbage was to be disposed of properly, garbage storage areas were to be kept sanitary, garbage containers were to be covered when moved to the disposal area, and pest control contractors were to come monthly. The policy also stated maintenance employees were to check for pests during walk-throughs, bug light trap pads were to be changed every Tuesday, and maintenance was to be notified when any pest or insect was reported. During observations, surveyors noted flies in multiple areas of the facility, including Hall A, Hall B, Hall C, and Hall E. Staff interviews confirmed the issue had been ongoing, with an RN stating she had noticed numerous flies during April and May and a few in June, an LPN stating flies had been present in residents’ rooms, another LPN stating flies had been noted in the facility and residents’ rooms, and housekeeping stating the flies had been bad and that about 20 flies were counted in one day. Surveyors also observed flies around the smoker’s patio and saw one fly enter the facility through an automatic door as residents entered. Surveyors observed conditions outside Hall C’s exit door near the dining room that included uncovered gray garbage bins and loose trash items such as used gloves, empty water bottles, loose papers, and an open carton of protein supplement not contained in garbage bags. Flies were observed around the exit door and landing on the cement, and a hanging fly strip was full of dead flies. Maintenance stated the door was opened several times a day to bring trash to the dumpsters and acknowledged the bins should not have loose garbage in them and should be covered. The contracted pest control staff stated he had provided monthly service for crawling insects only, had seen only a few flies during his visit, and had previously suggested fly bait to the Administrator, but no response was given. The DON stated flies were bad inside and outside the facility in April and May and were still present, and the Administrator stated the facility should have maintained a pest free environment.
