Kitchen Food Storage and Sanitation Deficiencies

Mills Nursing & RehabilitationMayfield, Kentucky Survey Completed on 05-21-2026

Summary

The facility failed to store and prepare food under proper sanitary conditions and failed to ensure food preparation equipment was clean. Facility policies required refrigerated, frozen, and dry food items to be stored properly, covered, labeled, dated, and kept clean and dry, and required food contact surfaces and dining areas to be cleaned and sanitized after each use. Daily cleaning lists also assigned staff to clean the steam table, contact surfaces, fryer, and equipment each shift. During the kitchen tour, surveyors observed a dented can of refried beans stored with other canned goods, a build-up of grease, dirt, dust, and food particles on the fryer, and a roll-up door separating the dining room from the kitchen with food, dust, and dirt build-up. The steam table was observed with large amounts of sticky yellow substance, grease, dirt, and food particles on the front area, bottom shelf, and temperature regulating knobs. Staff confirmed the dented can was in the wrong place and removed it during the survey. Later observations showed the same sanitation problems remained, including food splatter and dirt on the roll-up door, sticky discolored substance and food particles on the steam table, and food droppings and burnt areas in and around the fryer. Surveyors also observed an opened box of raw hamburger patties in the walk-in freezer with patties showing variation in pink, brown, and light tan colors around the edges. Facility staff acknowledged the patties were not properly stored and that the box should have been sealed. Staff interviews indicated kitchen cleaning responsibilities were assigned, but the observed build-up had not been addressed.

The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.

The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
