Infection Control Failures in Droplet Precaution Room

Summary

The facility failed to maintain an infection prevention and control program designed to provide a safe, sanitary, and comfortable environment and to help prevent the development and transmission of communicable diseases and infections. Review of the facility’s transmission-based precautions policy showed that anyone entering a resident’s room on droplet precautions should perform hand hygiene and don a face mask, and that additional PPE such as a gown, gloves, face shield, and/or goggles should be used when there was a substantial risk of exposure to mucous membranes or respiratory secretions. The infection prevention and control policy stated the program was based on national standards and guidelines to prevent, recognize, and control infection and communicable disease. Observation of a resident’s droplet precaution room showed the outside door left open and no clear curtain pulled around the bed during multiple observations. The Scheduler entered the room with lunch trays without donning PPE and then entered another resident’s room and another resident’s room again without PPE. An LPN rolled a treatment cart into a resident’s room to complete a treatment and then left the room without cleaning the cart. During interview, the LPN stated she did not feel the cart required special cleaning after being in the room. The IP stated staff entering droplet precaution rooms should close the door or pull the clear curtain around the resident, don PPE including at least a gown, mask, and gloves, and clean any cart brought out of a resident’s room with disinfectant. The DON stated all staff had to wear PPE when entering and exiting a resident’s room on droplet precautions, and the Scheduler stated she did not pay attention to the droplet precaution sign and did not wear PPE when delivering trays.
