Summary

The facility failed to ensure that one resident’s admission MDS assessment was completed within 14 days of admission. The resident was admitted on [DATE], and the admission MDS had an ARD of 10/21/2025, but the V200B date showing completion of the CAA process was not signed by the MDS Coordinator until 11/05/2025, which was the 22nd day of admission. Review of the resident’s census tab and assessment record confirmed the delay in completion of the admission assessment. During interview, the MDS Coordinator stated she was responsible for signing the admission MDS as complete and confirmed it was due no later than the 14th day of admission. She said the delay occurred because she was waiting for Social Services staff to complete portions of the assessment, and the related data was not locked in the system until 11/03/2025. SS staff confirmed that the assigned portion was opened on 10/27/2025 and signed on 11/03/2025, and explained that the total MDS could not be completed and locked until all portions were done. The DON stated she was unaware of issues with late MDS assessments or Social Services not completing and/or locking their portions in a timely manner, and the Administrator stated he was unaware of any late assessments prior to surveyor intervention.
