Delayed Response to Exit Door Alarm Allowed Resident Elopement

Summary

The facility failed to provide adequate supervision and failed to respond appropriately to a door alarm when a cognitively impaired resident with a history of wandering and exit seeking eloped from the Blue-hall emergency exit door. The resident had diagnoses including dementia, severe cognitive impairment on the BIMS, a history of restlessness and agitation, and was identified as being at risk for falls. Her care plan documented supervision needs for walking and transfers, and her elopement risk assessment documented wandering, exit seeking, altered perception, restlessness, and dementia-related confusion. On the night of the incident, the resident pushed on the delayed egress exit door, the alarm sounded immediately, and the door opened after about 15 seconds. A licensed nurse was seated at the nurse’s station with a clear view of the door but did not look up or respond when the alarm sounded. The alarm continued while the nurse remained at the computer. A CNA later asked where the alarm was coming from, and the nurse said it was likely an exit door. CNAs then searched for the source, identified the Blue-hall exit door, and later found the resident outside in front of the building and brought her back inside. The resident was outside in the dark for approximately 25 minutes in about 60-degree weather. Facility review and interviews documented that the nurse did not initiate an immediate response when the alarm first sounded and did not promptly check the door or outside area. Video review showed the resident exiting through the door while the nurse remained at the desk and did not react to the alarm. The report also states the resident had no resident-specific interventions in her care plan to direct staff during wandering and exit seeking behaviors, and the facility determined the deficient practice constituted immediate jeopardy.
