Failure to Use Enhanced Barrier Precautions During Resident Care

Summary

The facility failed to utilize Enhanced Barrier Precautions (EBP) during direct care for residents with wounds and a urinary catheter. One resident was observed in bed with the urinary catheter collection bag directly on the floor and containing approximately 400 mLs of dark brown, tea-colored urine. The room door displayed EBP signage indicating gown and glove use for contact with wounds, urinary catheters, and infections, yet staff handled the catheter bag and emptied it without wearing a gown, and one staff member held the bag in an ungloved hand before sanitizing and applying gloves. A second resident had clean, intact dressings on both lower legs and an uncovered open lesion with bright red blood and raised skin above the dressing on the right leg. The resident stated staff used gloves during leg care but did not wear gowns. Staff entered the room, sanitized hands, and applied gloves, but did not apply gowns despite confirming that PPE should be used. During a transfer with a full body lift, two CNAs again entered without gowns and only applied them after being questioned. The resident was also observed bleeding above the right lower leg dressing during the transfer. During wound care, an LN changed the resident’s lower leg dressings without sanitizing scissors before use, placed the scissors blade directly under the dressing in contact with the leg, and did not place a dressing over newly observed open areas. The LN also failed to remove gloves and sanitize hands between tasks, placed a bloody dressing on the bed beside the resident’s leg, retrieved clean supplies with the same gloves, and did not sanitize hands or change gloves between care of each leg or before applying a dressing to the new wound area. Therapy staff also reported providing upper body strengthening and range-of-motion modalities to the resident without using EBP gowns because the wounds were on the lower legs. The facility’s policy stated EBP should be used for residents with wounds that should be covered and during transfers, therapy gym activities, and close physical contact.
