Summary

The facility failed to ensure Resident #21 received alprazolam as ordered and did not reorder the medication or use emergency stock to prevent missed doses. Resident #21 had a diagnosis of anxiety disorder, was cognitively intact with a BIMS score of 15 out of 15, and had a care plan addressing use of anti-anxiety medication. The physician order in the EHR was for alprazolam 0.5 mg, 1 tablet by mouth twice daily for anxiety. The June 2026 MAR showed multiple doses marked with a chart code indicating "Other/See Nurses Notes," and nursing notes documented the medication was "on order" on 6/1, 6/2, and 6/4, and "out of medication" on 6/3. During interview, Resident #21 stated there had been an issue with her medications running out and she had to wait 3 or 4 days before receiving it. Staff interviews showed the medication was not reordered in time and the pharmacy had not received a refill request between the prior order and the new order placed on 6/4/26. Staff also stated the resident was out of alprazolam for a couple of days, and the DON stated she believed the medication should have been available in emergency stock and that staff should have checked it and contacted the pharmacy when the medication did not arrive. The facility policy required routine and emergency medications to be obtained in a timely manner and directed nurses to reorder medications when six or fewer doses remained.
