Unsafe Food Handling and Cross-Contamination During Meal Preparation and Service

Summary

The facility failed to ensure dietary staff prepared and served food using safe hygienic practices to prevent cross-contamination during meal preparation and meal service. During observation, Staff R and the Dietary Manager prepared meals while wearing gloves, but Staff R repeatedly handled food, utensils, drawers, refrigerator doors, lids, and other surfaces without changing gloves or washing hands. Staff R removed lids with a gloved hand, used tongs to place chicken-fried steak patties into a blender, carried broth across the kitchen in a spoon to add to the blender, and continued handling food and equipment with the same pair of gloves throughout the entire observation. Staff R also used a gloved hand to pull bread slices from a loaf and placed them into a blender with milk to puree them. During lunch meal service, Staff R continued using the same gloves while taking lids on and off food containers, serving food with tongs and spoons, and touching a plastic-coated menu on the steam table while plating each resident meal. Staff R reached into a refrigerator behind the steam table to retrieve barbeque sauce, squirted it onto a resident's plate, and returned it to the refrigerator. Staff R also reached into a loaf of bread with a gloved hand to remove slices for residents and retrieved butter patties from the refrigerator while serving meals. The Dietary Manager stated staff needed to wash hands when going from the dining room to the kitchen, before putting on and after taking off gloves, and to change gloves after touching food, their face or body, or surfaces such as refrigerators and drawers. The facility policy stated staff shall adhere to safe hygienic practices to prevent contamination of foods from hands or physical objects and shall not touch food with bare hands.
