Resident left in wheelchair overnight and found wet and slouched

Summary

The facility failed to ensure one resident who was dependent for transfers was repositioned and kept clean and dry. The resident’s MDS listed multiple diagnoses including cancer, hip fracture, diabetes, anxiety, and depression, and coded the resident as always incontinent of bowel, incontinent of urine, and dependent on transfer to chair, bed, or toilet. The care plan required assistance of two staff with a mechanical lift using a purple sling for transfers. On the night of the event, the resident remained in a wheelchair for an extended period and was later found slouched down with feet off the wheelchair pedals and clothing visibly soaked with urine. Staff interviews and statements indicated the resident had been in the wheelchair since the evening meal and through the night, with differing accounts about whether the resident refused to go to bed or whether staff failed to follow through with repositioning and incontinent care. One CNA reported assisting the resident to bed and changing her around 3:30 AM, then placing her back in the wheelchair after the resident said she wanted to return to the chair. Another CNA and an RN later confirmed the resident was still in the wheelchair in the morning, wet, and had not been laid down overnight. By morning, staff documented that the resident was slouched over in the wheelchair, the front of the brief and pants were wet, and the back of the pants was very wet. One CNA stated the resident had been in the chair since dinner and that even the transfer sling was drenched. The DON stated staff were expected to do walking rounds at shift change, report refusals for positioning or incontinent care, and reposition residents every couple of hours with brief changes as needed. The facility policy on turning and repositioning directed staff to assist residents into proper alignment, and the rights and responsibilities document stated the facility would strive to assure each resident a dignified existence and self-determination with communication.
