Summary

The facility failed to provide appropriate pressure ulcer care and to prevent a new pressure ulcer from worsening for a resident with significant risk factors, including quadriplegia, multiple sclerosis, peripheral vascular disease, malnutrition, and dependence on staff for repositioning and transfers. The resident’s care plan identified impaired skin integrity, impaired mobility, and episodes of incontinence, and later noted that she preferred to direct her own care and sometimes refused repositioning or incontinence care. The resident was documented as cognitively intact on the MDS, but also as dependent on staff to roll left and right and to transfer from surface to surface. A new pressure injury to the right buttock was documented as a Stage II and was acquired in the facility. The record showed the wound was first identified during an assessment after the resident had received Z-guard to the peri-rectal area earlier that same day. Facility documentation then failed to show a skin check until later, when the wound was documented as a Stage III. The resident was also noted to have been in bed when the wound was discovered, and the facility’s documentation showed repeated gaps in repositioning records. The Documentation Survey Report for January 2026 showed 63 of 93 shifts with no documentation that the resident was repositioned, including shifts marked as not completed or with no entry. The February 2026 report showed 53 of 84 shifts with no documentation that repositioning occurred. The treatment record also showed eleven missed wound treatments, with most having no reason documented. The resident’s wound care notes described ongoing cleansing and autolytic debridement, but the wound later deteriorated, prompting imaging and then transfer to the hospital for evaluation and treatment. Hospital records showed the sacral ulcer had extended to bone, with osteomyelitis and MRSA infection identified, and the resident underwent debridement and bone biopsy while hospitalized.
