Medication Administration Errors and Improper Insulin Pen Use

Summary

The facility failed to ensure residents received medications as ordered and according to the six rights of medication administration. Resident #31 had diagnoses of Alzheimer’s disease and anxiety disorder, with impaired short-term and long-term memory and severely impaired decision-making skills. The record showed the resident was to receive prescribed medications per the physician’s orders, but the event history documented multiple medication errors involving this resident, including receiving the wrong medication and receiving insulin without an order for that insulin. On 2/4/26, Resident #31 was given 0.5 mg of lorazepam instead of 0.25 mg of oxycodone during the noon medication pass. The narcotic count later showed Staff L, RN, signed out Resident #31’s oxycodone but administered one of Resident #39’s lorazepam doses instead. On 3/23/26, Resident #31 received 23 units of glargine insulin despite having no order for glargine; the provider then ordered blood sugar checks every hour for 24 hours and PRN. On 4/19/26, Resident #31 again received morphine instead of scheduled lorazepam at noon. Staff F, CMA, stated she gave morphine from the cart for the AM and noon passes instead of the lorazepam stored in the refrigerator. Resident #5 had diabetes and a BIMS score of 10, indicating moderate cognitive impairment, and was receiving insulin routinely. During observation, Staff B, RN prepared and administered the resident’s insulin glargine pen but did not clean the rubber seal or prime the pen before use. The insulin pen instructions and the facility’s insulin pen policy both directed staff to clean the seal and perform a needle safety test/prime the pen before administering the dose. The DON stated staff were expected to follow the manufacturer’s guidelines for insulin pens and to prime them before giving insulin.
