Summary

The facility failed to ensure that one resident with moderate cognitive impairment and therapy-identified mobility limitations had the appropriate staff assistance when using the elevator. Resident #4 had an MDS BIMS score of 11 out of 15, was dependent on staff for transfers and cares, and had therapy documentation stating that the resident could maneuver an electric wheelchair with modified independence except for the elevator, where assistance was recommended for operating the doors and buttons. The care plan also noted a history of bumping into things and included directions related to mobility and reassessment. On 9/17/25, while a Chaplain Assistant was supervising residents going to chapel, Resident #4 entered the elevator in an electric scooter and attempted to turn around and exit independently. The Chaplain Assistant was also assisting another resident in a manual wheelchair and, after holding the elevator door open, removed her hand from the door track opening as Resident #4 began exiting. The elevator door then went into nudging mode and contacted Resident #4’s left leg. The resident immediately reported pain, and staff later documented swelling and severe pain in the left lower leg. The resident was sent to the ED, where a large hematoma to the left lower leg was identified and x-ray showed no fracture. Following the incident, nursing notes documented persistent swelling, bruising, and severe pain, and later records showed the injury remained unhealed months afterward with ongoing wound care and pain during treatment. Interviews with staff showed that therapy had communicated the elevator-assistance recommendation to nursing, but multiple staff members stated they did not know who was responsible for helping the resident with elevator use. The Chaplain Assistant stated the resident took several minutes to turn around in the elevator before the door closed on the leg, and the previous Administrator stated that anyone could help with the elevator buttons, while also confirming the resident could not reach them.
