Failure to Use Gait Belt During Assisted Transfer and Ambulation

Summary

The facility failed to ensure a safe transfer and ambulation with the use of a gait belt as identified in the resident’s care plan during a staff-assisted transfer and walk from the bathroom to a recliner. Resident #1 had moderately impaired cognition, used a walker and wheelchair, and required partial/moderate assistance for walking and transfers. Her care plan, mini care plan, and the facility’s cheat sheet all directed staff to provide one to two staff assistance with a gait belt for transfers and ambulation, and the facility’s gait belt policy required direct care staff to use a gait belt whenever hands-on assistance was needed. On the day of the incident, an agency CNA assisted the resident back to her recliner after bathroom use without using a gait belt. The CNA reported that the resident was walking quickly, turned too fast toward the recliner, lost her balance, and began to fall. The CNA stated she placed her arm behind the resident and tried to ease her to the ground. Staff later found the resident on the floor, and the LPN asked where the gait belt was; the CNA stated she had not used one. A gait belt was then placed on the resident and three staff members assisted her to her recliner and then to bed. After the fall, the resident reported pain in her right hip area, her right leg appeared shorter, and she could not fully straighten the leg. She was sent to the hospital, where records documented an acute displaced fracture of the right femoral neck. She was transferred to another hospital because orthopedic coverage was unavailable, and she underwent hemiarthroplasty. The resident later stated her pain at the time of the fall was 7 or 8 out of 10. Facility staff and therapy staff confirmed that her transfer status at the time required contact guard assistance with a gait belt, and the DON stated the root cause was that the CNA failed to use a gait belt as indicated on the mini care plan.
