Failure to Use Individualized Bed Positioning Interventions for a High-Fall-Risk Resident

Summary

The facility failed to implement individualized interventions related to positioning in bed for a dependent resident who was at high risk for falls. The resident had diagnoses including seizure disorder, anoxic brain damage, cognitive communication deficit, and severe cognitive impairment. The resident was dependent for all ADLs, required staff assistance for transfers, and had documented fall risk scores that ranged from moderate to high. Observations and staff interviews showed the resident’s bed had been kept at waist height rather than in the low position before the fall, and staff stated the resident leaned to the right when positioned on her back with the head of bed elevated. Staff also stated the resident could make slight, unpredictable movements and that wedges were available to position her and establish boundaries, but these were not in use before the fall. After the incident, the care plan was updated to include low bed, floor mats, scheduled repositioning, and wedges, and the interim DON stated staff had not completed the fall risk assessments correctly and that education on the changes had not yet reached all staff.
