Unlicensed CNA Involved in Medication Administration and Narcotic Counts

Summary

The facility failed to ensure that only licensed staff administered medications when an agency LPN directed a CNA to assist with medication administration for a resident who was refusing medications. During an interview, the CNA stated the LPN handed her the medication cup, stood in the doorway, and pressured her to help give the medications. The CNA attempted to administer the medications with a spoon, then reported that the LPN crushed the medications and gave them back to her, after which she returned them when the resident still would not take them. Another CNA present in the room confirmed that the LPN asked whether the CNA was a nurse and then told her to give the medication. The facility also failed to ensure that narcotic counts were completed only by qualified personnel and that licensed nursing staff completed the shift-to-shift count together. A CMA observed a CNA performing a narcotic count with an LPN and reported this to the ADON, who confirmed the CNA was not a med aide. The CNA stated she was told by an LPN that she had to count narcotics, despite telling him she did not have a license and could not do so. She reported that she counted with one LPN and then separately with another LPN, rather than the nurses completing the count together. A second LPN confirmed that the CNA performed a narcotic count and received a nurse report, and stated the CNA should never have been placed in that position or left with the cart keys. The narcotic and controlled substance form showed the CNA’s initials for the shift. The DON stated that a CNA must never work beyond scope of practice and should not perform a narcotic count or administer medication, and the Administrator stated that only nurses may administer medications and that nurses must count narcotics together at the end of every shift.
