Summary

The facility failed to provide safe, appropriate pain management for a resident with intact cognition who had diagnoses including peripheral vascular disease, arthritis, malnutrition, asthma, and dislocation of the left hip. The resident’s MDS documented that he experienced occasional pain and rated his worst pain in the past five days as 7 out of 10. During interview, he stated he had a lot of pain when getting in and out of bed, and staff interviews later confirmed that he complained about hip pain and pain during care and transfers. The resident had an order for oxycodone 5 mg by mouth twice daily starting 5/7/26, with an additional every-8-hours as-needed order for pain. The MAR showed multiple missed scheduled doses, including both doses on 5/8/26, both doses on 5/9/26, both doses on 5/10/26, and the morning dose on 5/11/26. On 5/11/26, the resident rated his pain as 6 out of 10 when he finally received the medication at night, and later that day he received an as-needed dose for pain rated 9 out of 10. On 5/13/26, he reported worsening pain with ratings of 8 out of 10 in the morning and 7 out of 10 in the evening. The missed doses occurred because the pharmacy did not receive a written prescription after the physician gave the order, so the prescription could not be filled even though the order remained on file. Nursing notes documented that staff contacted the pharmacy after the resident did not receive his narcotic medication and that the pharmacy reported there was no current prescription. Staff also documented that they were waiting for an emergency prescription before the medication could be obtained. The resident’s care plan identified pain related to chronic wounds and disease process and directed staff to assess pain, give medications as ordered, and notify the physician of uncontrolled pain, but staff did not follow those interventions when the ordered pain medication was not provided.
