Unsafe Recliner Footrest Handling Caused Resident Leg Injury

Summary

The facility failed to ensure safe handling of equipment during resident assistance when a CNA attempted to lower the footrest of a recliner for a resident who had requested help because she felt weak. The resident reported that the CNA used her foot to push the footrest down and struck her right lower leg twice during the attempt. The CNA stated she had difficulty lowering the footrest, pushed it with her foot, and said any contact with the resident was unintentional. The Administrator stated the recliner footrest was expected to function properly and that staff should remove blankets, ask for help, and notify maintenance when a footrest was not working. After the interaction, the resident complained of severe right lower leg pain and was found seated in the recliner with a pale, grayish, tender hematoma on the lateral anterior aspect of the right lower leg. The hematoma began actively bleeding during assessment, with an estimated blood loss of 300 mL, and direct pressure did not stop the bleeding. EMS was called, and during transfer to the stretcher the skin over the hematoma split open, requiring additional dressings before the resident was transported to the hospital. The resident stated the CNA had kicked or struck her leg while trying to lower the recliner footrest, and another CNA later observed a large gray-colored hematoma on the right lower leg. Hospital records documented a large soft tissue hematoma of the right lower leg, lymphedema and venous stasis of the lower legs, severe obesity with a BMI of 40.0-44.9, edema in both lower legs, and a history of pulmonary embolism. The resident was taking Eliquis, and her hemoglobin dropped to 6.6, requiring a total of 3 units of packed red blood cells. The facility policy on safe lifting and movement of residents required staff to incorporate resident safety and medical conditions into care decisions, use equipment safely, and ensure equipment used for resident movement remained in good working order.
