Hand Hygiene and PPE Failures During Incontinence and Catheter Care

Summary

The facility failed to complete appropriate hand hygiene for two residents during observed incontinence care, and failed to apply appropriate PPE during catheter care for a resident on Enhanced Barrier Precautions (EBP). Resident #3 had severe cognitive impairment with a BIMS score of 3, was dependent for toileting hygiene and personal hygiene, had an indwelling catheter, and had bowel/bladder incontinence. During one observation, multiple staff members assisted Resident #3 with transfer and incontinence care, with several instances where gloves were removed and hand hygiene was not performed before continuing care, before leaving the room, or before applying a new brief. During the same care episode, staff also completed catheter-related care after the resident voided with the catheter in place, and no gowns were applied despite the resident being on EBP and the DON stating gowns were expected when the catheter was leaking. A second observation of Resident #3 showed staff again performing incontinence care and transfer activities with repeated breaks in hand hygiene and glove changes. Staff cleansed the resident’s peri area and buttocks, applied a new brief, repositioned the resident, and handled bedding and equipment, but failed to change gloves or perform hand hygiene between tasks and before leaving the room. The DON stated staff must complete hand hygiene and a glove change after personal care and before applying a clean brief, and must perform hand hygiene before leaving a resident’s room. Resident #4 also had severe cognitive impairment with a BIMS score of 1 and was dependent for toileting hygiene and personal hygiene with bowel/bladder incontinence. During observed incontinence care, staff performed hand hygiene and applied gloves at the start, but then cleansed the resident’s peri area and buttocks, applied a new brief, repositioned the resident, and handled the mechanical lift and sling with failure to perform hand hygiene or change gloves between tasks. Staff also removed gloves and left the room without hand hygiene after the care was completed.
