Another resident stated he heard the food truck would not deliver because the facility owed money, and that the kitchen had been throwing things together and not writing meals on the board because staff did not know what they would have until the day of. Observations showed residents eating meals that did not match the facility-provided menu, including ham and bean soup with cornbread, stuffed green peppers with mashed potatoes, and corn dogs with sweet potato puffs. Facility staff confirmed the lack of consistent food deliveries and menu planning. The Dietary Manager stated the facility lacked a weekly menu, was worried about having enough food, and reported the food delivery company had suspended service due to an unpaid balance. She stated the facility lacked a dietitian to review daily menus since April and that she prepared meals based on available inventory without planning. The DON and Administrator both stated staff sometimes had to buy groceries because deliveries were not occurring, and the Administrator confirmed there were times staff and the CEO went grocery shopping. The food delivery company representative stated the facility had missed three to four deliveries because of an outstanding balance, and the Administrator later stated the facility lacked a formal policy for following menus reviewed by a dietitian.
