Failure to Provide Respectful and Dignified Care
Summary
The facility failed to ensure residents were treated in a respectful and dignified manner for 1 of 4 residents reviewed for resident rights. Resident B had diagnoses including left hip osteoarthritis and cerebral infarction without residual effects, and the quarterly MDS dated 6/16/26 showed impairment in both lower extremities, substantial to maximum assistance needed for toilet hygiene, toilet transfers, and dressing, and moderate cognitive impairment. Resident B stated that on the evening of 6/19/26, CNA 1 assisted her off the toilet but did not position the wheelchair as requested, repeatedly asked if she needed help, turned off and took away the call light after the resident used it to request someone else, and attempted to make her beg for assistance. The resident became upset, tried to get into the wheelchair, fell into it, and then refused CNA 1's help with returning to bed. Resident B said she was afraid of CNA 1, did not want her to return to her room, and reported the incident to staff later that evening. CNA 2 and the resident's family member both confirmed the resident was crying and upset about how CNA 1 had treated her. Resident D had diagnoses including cerebral infarction and legal blindness, and the significant change MDS dated 4/7/26 showed the resident required supervision/touch assistance for eating and was dependent for all other ADLs, while remaining cognitively intact. Resident D stated that during the evening meal on 6/19/25, CNA 1 assisted with feeding but did not honor the resident's request for a towel to cover her chest while eating, telling her she would clean her up if she got messy. The resident also stated that she and her roommate preferred to keep the privacy curtain open unless receiving personal care, but CNA 1 repeatedly closed it despite their requests. CNA 2 confirmed that Resident D always requested a towel during meals and that both Resident D and her roommate had complained about how they were treated by CNA 1. LPN 7 also observed Resident B upset and heard Resident D and her roommate upset about CNA 1's treatment of Resident D.
