Failure to Assess Skin Changes and Timely Document Medication Administration
Summary
The facility failed to assess skin related to a blackened toe for Resident G. Resident G had diagnoses including hemiplegia/hemiparesis following cerebral infarction, chronic kidney disease stage 3, severe protein calorie malnutrition, and adult failure to thrive. The care plan identified the resident as at risk for skin breakdown and directed staff to assess and document skin condition weekly and as needed, and to notify the physician of abnormal findings. However, the record review showed no skin issues on the most recent weekly skin assessment, no wounds in wound management notes, and no concerns noted during the shower assessment or transfer form before the resident was sent to an acute care facility. After transfer to the acute care facility, an RN documented that Resident G's left great toe was oozing pus from the nail bed and that the left third toe had a large black callous that was removed. The acute care wound assessment documented that the left third toe had a black callous that was removed, revealing pink new skin underneath, and that the left great toe nail bed was peeling off with purulent drainage coming from underneath the nail. The report also noted that the resident was referred for wound care and a podiatry consult was recommended for removal of the nail. The facility also failed to sign for medications immediately after administration for two residents reviewed for changes in condition. For Resident E, the May MAR showed three instances where medications were given but charted late, and an LPN stated she sometimes documented medication administration late even though she knew it should be documented immediately after administration. For Resident F, the May MAR showed medications documented as administered late on two occasions. In addition, the June MAR for Resident G showed medications documented as administered late nearly every day, and the Regional Director of Clinical Services stated medications should have been signed for directly after they were administered.
