Unsanitary Kitchen With Cockroach Infestation

Majestic Care Of GoshenGoshen, Indiana Survey Completed on 06-12-2026

Summary

The facility failed to maintain a sanitary kitchen in the main kitchen observed for sanitation. During an initial observation, live cockroaches were seen crawling out from underneath the refrigerator, under the stove, and under the stainless-steel food prep counter that housed a microwave. A bag of sleeved bowl lids was on the floor against the wall underneath the food prep counter near the cockroaches, and food and general debris covered in dust, along with grease buildup, was observed on the floor along the back of the oven. Dead unidentifiable bugs and dead cockroaches were also noted along the edges of the kitchen walls, including the dish room, and live cockroaches were observed by the refrigerator, behind the stove, by the serving steamtable, and along the kitchen walls. During interviews, the Dietary Manager stated she had not been aware of cockroaches in the kitchen and noted there had been a problem in the employee breakroom near the kitchen. The Assistant Director of Food Service stated cockroaches had been seen in the kitchen for the past year, and the Assistant of Dietary Services confirmed awareness of the cockroach issue for the past year and said she had informed the former Director of Maintenance about it about two months earlier. The Dietary Director stated staff had a daily cleaning form and that the floor and equipment should have been cleaned every night for food debris, dust, and general dirt. Pest control reports for the past year showed monthly preventive service, but reports dated 6/5/2026 and 6/10/2026 noted roaches in the employee break room, mop room, kitchen dish room, and drains, and April 2026 notes referred to debris in the dietary dish room.

The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.

The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
