Improper food storage and unsanitary kitchen conditions
Summary
The facility failed to ensure proper food storage and sanitary conditions in the kitchen, dish room, and walk-in refrigerator. During an initial kitchen observation with the Executive Director, standing water covered the dish room floor and extended into prep areas while dietary staff moved around it. The kitchen also had food crumbs, grease buildup, debris on the floor, and dirty equipment, including the deep fryer, stove area, and metal prep stations. Opened and undated food items were observed under prep areas and in storage, including an opened bag of grits, a box of salt with an opened hole, and food debris and unidentified dried liquid on shelves beneath the steam table. The walk-in freezer and walk-in refrigerator contained multiple improperly stored foods. Observations included unsealed bags of sausages and breaded food in the freezer, and in the refrigerator expired yogurt, expired milk, undated opened ham wrapped in original packaging, cooked sausages in an undated bag, opened heavy whipping cream with the spout open, opened pie and cake with pieces missing, an unidentified loaf-formed meat product wrapped in torn foil thawing in a metal pan, pudding dated 6/16, unidentified food in a plastic container with loose wrap and no date, and shredded cheese in an unsealed bag with no opened date. The floors throughout the kitchen and walk-in storage were sticky and slick with grease, and there was no visibly posted cleaning schedule during the observation. On a second observation, the dish room and kitchen no longer had standing water, but the floors remained sticky and slick, and kitchen appliances and prep stations still had food debris, dried liquids, and grease buildup. The deep fryer continued to have old food crumbs around the top and floating in the grease. The Executive Director stated the dishwasher was broken and that water leaked from the garbage disposal into a bucket that overflowed onto the floor. The Registered Dietician stated there were concerns with sanitation in the kitchen and food storage areas and that staff education still needed to be completed. The facility policy required food to be stored in a clean, safe, and sanitary manner and required a posted cleaning schedule, but the cleaning schedules were not provided during the survey.
