Summary

The facility failed to maintain required temperature logs for the walk-in cooler, refrigerator, and freezer, and surveyors found the only temperature tracking log posted was a January 2026 log with the last documented entry on 1/31/2026. On 2/1/2026 at 9:18 a.m., the Dietary Cook verified there was no February temperature tracking log affixed to the refrigerator. The Dietary Supervisor later confirmed that dietary staff were responsible for posting new monthly logs and tracking temperatures twice daily, but the required current log was not in place at the time of the survey observation. During the kitchen observation, surveyors found refrigerated foods stored without open or use-by dates and foods with expired dates. The Dietary Cook verified cheese partially wrapped in saran wrap had no open label date or expiration date, hamburger buns had no open or use-by date, and turkey breast sandwich meat and roast beef each had an open date of 1/28/2026 and a use-by date of 1/30/2026. The Dietary Supervisor also verified the milk, turkey sandwich meat, and roast beef temperatures were above normal refrigerated range, with readings of 46 degrees Fahrenheit, 48 degrees Fahrenheit, and 47 degrees Fahrenheit, respectively. Surveyors also found the walk-in cooler temperature was out of range, with a reading of 48 degrees Fahrenheit, and the cooling fan was blowing warm air. The Dietary Cook stated the walk-in cooler temperature had dropped because staff were frequently going in and out of it, and the Dietary Supervisor stated dietary staff had not notified her that the temperature was out of range. The Administrator later stated the monthly temperature log must be maintained for all kitchen refrigerators, coolers, and freezers, that food items must be labeled and expired or spoiled products discarded, and that improper temperature can cause food spoilage and health risks. HVAC repair records documented multiple service visits related to the walk-in cooler, and the facility policy required refrigerated foods to be stored at 41 degrees Fahrenheit or below with temperatures checked and recorded twice daily.
