Summary

The facility failed to maintain a sanitary kitchen environment for all residents in the facility. On 11/18/2025 at 10:00 AM, surveyors observed food debris on work surfaces in the kitchen, on the floors of the kitchen, dry storage, refrigerator, and freezer, and on the carts used to transport resident food trays to the units. A large pile of empty boxes and other garbage bags was also observed on the kitchen floor. The monthly cleaning list posted in the kitchen was dated October 1, 2025, and only four spaces had been completed; no sheet for November 2025 was observed. On 11/19/2025 at 11:00 AM, the Executive Chef stated the dietary department was short-staffed and could use another employee to run the dishwasher and assist with cleaning duties. He said it was important to have a clean kitchen to prevent food borne illness, and that he checked with staff to make sure cleaning had been done before employees left for the day, but he was not checking the cleaning list for completion of the tasks. The dietary cleaning policy with a revision date of 11/2024 stated the facility would store, prepare, distribute, and serve food under sanitary conditions, keep floors free of debris, use a clean-as-you-go technique, clean and sanitize work areas, and have staff check cleaning schedules daily to ensure tasks were completed.
