Failure to Follow Ordered Oxygen Therapy and Care Plan Requirements

Summary

The facility failed to provide safe and appropriate respiratory care for one resident, R33, by not following the physician-ordered oxygen liter flow, not providing humidification for oxygen use, not properly storing oxygen tubing when not in use, and not developing a care plan for oxygen use. R33 was admitted with diagnoses including acute on chronic systolic congestive heart failure, hypertensive heart disease with heart failure, chronic kidney disease stage 3, chronic obstructive pulmonary disease, and nicotine dependence. The MDS indicated cognition was intact. During observation, R33 was ambulatory, alert and oriented x3, and verbally responsive, while oxygen tubing and the nasal cannula were seen placed on the bed and not properly stored, the oxygen concentrator was running at 3L/min, and no humidification bottle was attached. R33 stated he had been using oxygen most of the time. The RN said oxygen tubing should be stored in a clear plastic bag when not in use and that a humidification bottle should be available, though she was unsure why it was not present. The DON stated oxygen should be administered as ordered by the physician, tubing and nasal cannula should be stored properly in a bag when not in use, humidification should be available and changed weekly and as needed or as ordered, and oxygen use should be care planned. The physician order summary showed an active order for oxygen with tubing and humidifier changes weekly and oxygen PRN via nasal cannula at 2L/min, but the electronic health record contained no care plan for oxygen use. The facility policy also stated that when oxygen is not in use, tubing and nasal cannula should be stored in a labeled plastic bag.
