Medication Storage and Held BP Medications Without Provider Notification

Summary

Medication administration did not follow professional standards when a registered nurse stored folic acid in a medication cup rather than in its original packaging and then administered it to a resident. The nurse stated she took folic acid from another medication cart because the resident’s cart did not have it, and later stated that medications should not be transferred from the original container and stored in a medication cup for administration. The facility policy stated medications are to be administered safely, medications are not to be prepared ahead of time, and medications should be documented after delivery. The nurse also held blood pressure medications for two residents without documented provider notification. One resident had diagnoses including acute on chronic diastolic congestive heart failure, atrial fibrillation, hypertensive heart disease with heart failure, nonrheumatic aortic valve disorder, cardiomegaly, peripheral vascular disease, atherosclerosis of native arteries of the right leg, and hypertrophic cardiomyopathy. After blood pressure readings of 81/54 and 104/75, the nurse held amlodipine besylate 10 mg daily, hydrochlorothiazide 25 mg daily, and losartan potassium 50 mg daily and said she would revisit the blood pressure after lunch. Another resident had a blood pressure of 112/48 with pulse 63, and the nurse held lisinopril 10 mg twice daily and metoprolol tartrate 25 mg twice daily, stating she would check the blood pressure again after lunch. The DON stated that when blood pressure or other medications are held outside parameters, the nurse should use nursing judgment and consult the doctor immediately for directions, and that if a medication is not given or is given late, the provider should be notified for additional orders. The report also states the nurse said she notified the doctor, but if it is not documented it did not happen. The physician order policy stated the physician or extender is consulted when changes in condition occur or when an alteration to prescribed treatments or medications is needed.
