Summary
The facility failed to obtain and monitor weights for a resident with recent significant weight loss and failed to inform the dietitian of decreased oral intake. The resident had multiple sclerosis, required staff assistance with all activities of daily living, and was cognitively intact. Her care plan identified her as at risk for weight loss. Her weight record showed a decline from 237.5 pounds to 218.9 pounds and then to 206 pounds, with no documented weight in October and no documented weights after 11/5/25. A dietary progress note documented that she had lost 13% of her weight in 3 months, was eating less than 50% of meals, and was skipping meals. The dietitian recommended a liquid protein supplement three times daily, weekly weights, and monitoring of oral intake. During the survey, the resident stated she had been losing weight and had not been hungry. Her meal intake record showed she primarily consumed 0-25% of meals and refused 12 meals during the reviewed period. When weighed in the surveyor’s presence, she weighed 199 pounds, showing an additional 3.4% weight loss from the last documented weight. The dietitian stated she had not been aware the resident had not been weighed since 11/5/25 and had not been notified that oral intake remained poor after her assessment and recommendations. The facility also failed to notify the dietitian that another resident’s nutritional supplement had been discontinued. That resident had diagnoses including paraplegia, dementia, and dysphagia and had experienced significant weight loss over time. The dietitian had recommended a 60 mL liquid nutritional supplement three times daily and weekly weights after staff noted reduced intake and significant weight loss. The MAR showed the supplement was given inconsistently and then discontinued when the resident refused it. The dietitian stated she was not aware the supplement had been discontinued and said she would have implemented other interventions if she had known, including other supplement options.
