Summary

The facility failed to notify the resident’s POA and NP after a fall and after a decrease in oxygen saturation for one of three residents reviewed for notification. The resident had diagnoses including vascular dementia, atherosclerotic heart disease, and COPD, and his records documented moderate cognitive deficits, high fall risk, and a care plan problem area related to falls and poor safety awareness. Nursing progress notes later documented that the resident complained of hip/femur pain after being gotten out of bed, and an LPN obtained stat x-rays and placed him in a recliner while awaiting imaging. The record did not contain documentation that the resident fell on the reported date or that the NP or POA was notified at the time of the event. A late entry noted that family arrived after being notified that he had fallen a couple days prior, but the note also stated the family had first been informed by a CNA that an agency nurse said he had fallen, though she described it as sitting down on the floor. The NP later documented that she was called about left hip pain and a possible fall over the weekend, and that staff were unaware of the fall until that day. The resident was sent to the hospital, where the ED record documented left hip pain after a fall and x-ray findings of an intertrochanteric fracture of the left femur with mild impaction. The resident also had an order for oxygen at 3 L/min via nasal cannula with oxygen saturation to be maintained at 90 percent or greater. Staff interviews described an episode in which the resident was found without portable oxygen, had an oxygen saturation in the 80s or 78, and improved after oxygen was applied, but the NP was not notified. The NP stated she should have been contacted when the oxygen saturation dropped below 90. The facility policy stated that changes in resident condition or treatment are to be immediately shared with the resident and/or representative and reported to and consulted with the attending physician, including significant changes and accidents resulting in injury with potential for physician intervention.
