The admission fall risk assessment documented the resident as high risk for falls. The facility’s Regulatory Investigations Policy stated that all employees are expected to cooperate with investigations and surveys conducted by regulatory bodies, and the Charting and Documentation Policy stated that all services provided to the resident or changes in condition shall be documented in the medical record. The resident matrix documented 45 residents living at the facility.
