Summary

The facility failed to provide timely ADL assistance to a dependent resident who required substantial to maximum assistance for transfers and was documented as needing a mechanical lift and two-person assistance. The resident’s record showed diagnoses including COPD, difficulty walking, weakness, chronic atrial fibrillation, left below-knee amputation, type 2 diabetes, chronic kidney disease, and cardiomyopathy. The care plan directed staff to transfer the resident with the assist of 1 and a gait belt in one problem area, and to provide extensive assistance with mobility and mechanical lift transfers in another, while the MDS documented the resident was cognitively intact with a BIMS score of 15. On the day of the event, the resident stated he was sitting in the doorway of his room with his call light on after an episode of bowel incontinence and needed to be changed. He reported that a CNA passed his room multiple times, and that two CNAs eventually came to his room but left without helping him. The resident stated he remained in bowel incontinence for about four more hours until the next shift came on and assisted him with toileting hygiene and getting into bed. Staff interviews confirmed the resident had been up in his wheelchair earlier in the day, requested to be laid down after lunch, and then waited until approximately 6:00 PM to 6:30 PM to be placed in bed. CNA interviews showed that staff recognized the resident required two-person mechanical lift assistance, but they delayed returning to him because other residents had been up longer and needed to be laid down first. One CNA stated the resident became angry and she and another CNA did not return to his room after the argument. Another CNA later assisted the resident into bed and reported he said he had been up for hours and complained he had not been changed in a while. The DON stated residents should be accommodated at their request if they are cooperative with staff providing care, and acknowledged that four hours was too long for the resident to wait. The facility’s Safe Patient Handling Program Policy stated resident care would be provided in a safe, appropriate, and timely manner in accordance with the resident’s care plan.
