Summary

The facility failed to maintain residents’ rights to dignity, respect, and timely assistance by not responding to call lights within an acceptable time for three of four residents reviewed for call light response time. The facility’s grievance logs for April, May, and June 2026 documented concerns about delayed call light response times, including complaints that call lights were not monitored and that response times exceeded acceptable limits. The facility policy revised 1/26 states residents are to be provided dignity and respect at all times and that staff are to respond appropriately to resident needs. On 6/15/2026, R3 reported waiting over 45 minutes for a call light to be answered. At 10:59 a.m., a call light was flashing at the nursing station with no CNAs visible nearby while two additional call lights were also activated. At 11:07 a.m., R8’s call light was sounding at the nurses station with a timer showing approximately 29 minutes. At 11:10 a.m., an LPN confirmed call lights should be answered within 5-10 minutes and acknowledged R8’s call light had been active for about 29 minutes. Also at 11:04 a.m., R4’s POA stated that although R4’s call light functions, staff response is frequently delayed beyond 30 minutes when assistance is needed for care.
